A wet patch under the chin at 3 am is not always disease, and it is not always nothing. A sleeping cat stops swallowing, so a relaxed jaw lets saliva pool and drip: a patch 1 to 2 cm across on the pillow is normal in warm weather. A patch 5 cm across, a string of drool 3 to 5 cm long, or bedding soaked through 2 nights in a row is a signal that needs a 2-week record and 2 specific checks.

What normal drooling looks like

An awake cat swallows roughly 1 to 2 times a minute, which clears 0.5 to 1 ml of saliva each time. In deep sleep that reflex slows, and a cat lying chin-down on a cushion leaves a damp spot 1 to 2 cm wide by morning. Three things stay normal: the patch does not grow past 3 cm, the cat wakes with a clean mouth and no smell, and weight holds within 100 g over 2 weeks.

The moment 1 of 3 numbers changes, the drool stops being cosmetic. A patch over 5 cm, more than 2 soaked spots per night, or a 5 percent weight loss in 2 months moves the case into the table below.

Three groups of causes

Sort by 1 question: is the drool worse when the cat is awake and eating, worse only in deep sleep, or worse with nausea and no mouth pain?

  • Mouth and teeth, the most common group. Gingivitis, gingivostomatitis, tooth resorption, a fractured tooth, an oral mass, or an ulcer under the tongue. Clue: bad breath scored 2 or 3 out of 3, drool that strings 3 to 5 cm, eating on 1 side, and a cat that drops hard food after 2 to 3 bites. Pain reduces swallowing, so the pillow gets wet before the owner notices the mouth.
  • Nerve, swallow, and airway. Trigeminal or facial nerve damage, laryngeal paralysis, a pharyngeal mass, myasthenia, a seizure with heavy salivation, or vestibular disease with a head tilt. Clue: drool on 1 side only, a lip that hangs, a meow that changed, or 1 episode of 30 to 90 seconds with paddling and dilated pupils.
  • Nausea and whole-body causes. Chronic kidney disease, liver disease, an overactive thyroid, diabetes, pain anywhere, heat at 30 C or more, or a toxin such as a lily, permethrin, or a cleaning agent. Clue: drool plus lip licking, 20 to 40 ml of water drunk at once, vomiting 1 to 3 times a week, or a cat that sits over the bowl without eating.

When to go: night-by-night grading

Drooling at night is graded by what the bedding looks like in the morning, not by how the cat looks at midnight.

What you findOther signsAction
Patch 1 to 2 cm, 1 nightNone, cat eats in the morning, 38.5 CPhotograph, log 7 nights, no visit yet
Patch 3 to 5 cm, 2 to 3 nightsBad breath, 1 sided chewingBook a dental exam within 7 days
Soaked bedding, every nightWeight down 100 to 200 g, dull coatExam plus blood panel within 48 h
Any drool, plus head tilt or lip droopWobbling, 1 pupil largerVet within 24 h; nerve signs do not wait
Drool plus paddling for 30 to 90 sDilated pupils, then confusion for 5 to 10 minEmergency today; a seizure is not a wait
Drool plus open-mouth breathing at 40 breaths or moreBlue or grey gums, cannot lie downEmergency now, any hour

Two rows carry most of the risk. Nerve signs and seizures both look like drool at 3 am and both need the same night, because a cat that cannot swallow is 1 vomit away from aspiration. The slow row is the other trap: 5 cm patches for 2 weeks with normal appetite is how chronic kidney disease and severe dental disease present, and both are cheap to confirm and expensive to ignore.

Two checks, in this order

First, a full oral exam with a periodontal probe under light sedation, plus full-mouth dental X-ray, 10 to 20 minutes and 150 to 400 dollars. Probing finds pockets 3 mm or deeper and X-ray finds resorption under the gum line that no eye can see, which is why a normal-looking mouth still needs film. Second, a blood panel with creatinine, SDMA, T4, glucose, and liver values, 20 minutes and 80 to 200 dollars. Those 2 tests separate the 3 groups above in about 1 visit, and neither is replaceable by the other.

The 2-week record: 7 columns

Two weeks is the shortest window that shows a trend instead of a bad night. Fill 1 line per morning.

  • Date and which night, with 1 photo of the pillow next to a coin for scale.
  • Patch size in cm, longest and widest, 1 to 10.
  • Drool string in cm at wake-up, 0 to 5.
  • Breath score 0 to 3, and gum color at the 1 minute mark after lifting the lip.
  • Appetite as a fraction of the normal bowl, plus 1 note on chewing side.
  • Weight in grams, same scale, same time, once a week.
  • Water drunk in 24 hours in ml, measured from a marked bowl; a 4 kg cat usually takes 160 to 240 ml.

On the way, and tonight: 3 things not to do

On the way to the clinic, do not do 3 things: do not tilt the head back to look inside the mouth, do not syringe water in to rinse, and do not reach a finger past the front teeth. Tilting a cat that cannot swallow sends saliva into the airway, and 20 ml of aspirated fluid is enough for pneumonia in 24 to 48 hours. At home, do not wipe the mouth with a cloth every hour, since that hides the string length and the smell you need to report; wipe once in the morning, photograph, then leave it.

After the checks: 5 steps

Book the dental cleaning if probing found 3 mm pockets, since 2 weeks of antibiotics will not fix a tooth. Change to wet food at room temperature for 5 to 7 days if chewing hurts, and warm it to 30 C for smell, not for heat. Recheck weight at 2 and 6 weeks, because 100 g back is the first sign that the mouth is better. Keep the nightly photo for 4 more weeks after treatment, so a relapse shows in 3 days instead of 3 months. For drool that comes with trembling in sleep, compare with trembling in sleep; for loose stool at the same time, read yellow diarrhea; for plants, flea products, or cleaners, check the toxicity list before the drool is 1 night old.

Frequently asked

Is drooling in sleep ever normal in a cat?

Yes, when it stays small. A patch 1 to 2 cm across, no smell, a clean mouth on waking, and stable weight over 2 weeks is a relaxed jaw plus slowed swallowing, and warm rooms make it worse. Log 7 nights and compare the photos; a patch that grows past 5 cm or soaks the bedding 2 nights running is no longer in the normal range.

Which 2 tests actually find the cause?

A full oral exam with a periodontal probe under light sedation plus full-mouth dental X-ray, and a blood panel with creatinine, SDMA, T4, glucose, and liver values. The first finds resorption and pockets 3 mm or deeper that look normal from outside, the second finds kidney, thyroid, and liver disease behind nausea. Both take about 1 visit and cost 230 to 600 dollars together.

My cat drools and also twitches while asleep. Which problem comes first?

The twitching. Drool plus paddling for 30 to 90 seconds, dilated pupils, or confusion for 5 to 10 minutes afterwards is a seizure pattern and needs a same-day visit, because 1 seizure can be a toxin or a brain lesion and 3 in 24 hours is an emergency. Pure drool with a normal exam can wait 7 days; a seizure cannot.
