A 13 year old cat that urinates beside the box is usually reporting a body problem, not a behaviour problem. About 40 percent of senior house soiling cases in clinic practice trace back to pain, stiffness or a medical driver, and the rest respond to geometry: a lip of 5 cm or less, a box within 5 m of the sleeping spot, and 1 more tray than you think you need.
Spend 48 hours on medicine first
Before you buy a new tray, get 6 things checked: a urinalysis with specific gravity and sediment; a urine culture, even when the sample looks dilute; glucose and fructosamine; T4; creatinine plus SDMA; and blood pressure. Add a hands on joint exam and, if the cat resents hip extension, radiographs of hips and stifles. A senior panel plus culture usually lands between 120 and 260 dollars and answers the 4 questions that change the plan: infection, diabetes, thyroid, kidney. Treating cystitis with 7 to 14 days of an approved antibiotic will not fix a cat that cannot step over a 15 cm lip.
Why 40 percent is a mobility number
The 40 percent figure is not a personality trait; it is osteoarthritis, and some estimates put radiographic joint disease in 9 out of 10 cats past 12. A cat with sore hips does not squat low enough, so urine lands over the edge, or it avoids the box because the entry hurts. Watch the posture: a cat that stands while urinating, leans on the box wall, or exits fast is telling you the litter surface hurts. Sixty percent of these cats improve on pain control alone, usually within 10 to 14 days, which is why the vet visit comes before the behaviour plan.
The 5 m rule and the count rule
Distance is the most common fixable fault. Put a tray within 5 m of every place the cat sleeps, on every floor of the house, and keep 1 more tray than the number of cats. A cat with arthritis will not walk 12 m and down 1 flight of stairs at 3 am; it will use the rug at 2 m. Keep trays out of cupboards, away from washing machines and boilers, and never beside the food bowl. At least 1 tray per household should be uncovered, because a hood traps odour and forces the cat to crouch.
Lip height, floor area, litter depth
A senior tray wants a lip of 5 cm or less, an interior of at least 60 by 45 cm for a 4.5 kg cat, and 3 to 5 cm of unscented fine clumping litter. The old high sided box with a 15 to 18 cm wall is the single most common cause of misses in cats over 12: the cat has to clear 3 times the height it cleared at age 5. Cut a 12 cm doorway into the side of an existing box instead of buying new, and put a 30 by 40 cm mat in front so the cat does not slip on tile.
The 3 day position log
Log every event for 72 hours before changing anything, because the pattern tells you which fix matters. Write 6 fields per line: date and time, exact location to the metre, posture (squat, stand, spray), volume (small, normal, large), what was in the box (clean, 2 scoops of urine, full), and what happened 30 minutes before (meal, play, visitor, nobody home). Nine or more lines in 3 days is a real pattern; 1 line is noise. Add a 30 second blacklight pass at night, since old urine glows where you cannot smell it.
Fixes, in the order that pays
| Change | Before | After | Where to measure | Typical response |
|---|---|---|---|---|
| Lip or entry height | 15 to 18 cm | 5 cm, or a 12 cm door cut in | Inside wall of the tray | 3 to 7 days |
| Interior floor | 45 by 35 cm | 60 by 45 cm | Litter surface, not outer shell | 5 to 10 days |
| Distance from bed | 8 to 12 m, 1 floor down | Under 5 m, same floor | Tape from pillow to tray | 1 to 3 days |
| Tray count | 1 for 2 cats | 3 trays total | 1 per cat plus 1 | 2 to 7 days |
| Litter depth | 8 to 10 cm | 3 to 5 cm, unscented | Depth at the centre | 2 to 5 days |
Diabetes and the other medical drivers
Volume is the tell. A cat passing more than 40 to 60 mL per kg a day, drinking over 100 mL per kg, or producing heavy clumping balls in 24 hours needs glucose and a urine specific gravity that day. A blood glucose above 250 mg/dL with glucose in the urine points to diabetes; a T4 above 4.0 ug/dL points to thyroid; a specific gravity stuck under 1.020 points to the kidney. Diabetes is manageable with insulin and 2 fixed meals, but only after the diagnosis, and untreated diabetes at 400 mg/dL is a same week appointment.
Incontinence is not the same as soiling
If urine leaks while the cat sleeps, or the cat leaves a puddle without squatting, that is incontinence and it needs a different plan; the details are in cat incontinence. A male cat that crouches for 5 minutes, produces 0 to 3 drops and cries is an emergency within 12 hours, because a urethral plug can kill in 24 to 48 hours; read blockage signs before you wait for morning. Spraying on vertical surfaces at 20 to 30 cm height with the tail up is marking, not a litter fault.
Clean the old spots or the plan fails
Cat urine leaves uric acid crystals that survive normal detergent, and a cat will return to a spot that still smells like itself. Use an enzymatic cleaner, saturate 2 times the stain diameter, wait 10 minutes, blot, then let it air dry for 24 hours. Do not use ammonia or bleach, which read as urine and as a threat. Cover the treated spot with foil or a tray for 2 weeks, and recheck with the blacklight on day 7 and day 14.
Start with these 5 moves
- Book the vet within 48 hours for urinalysis, culture, glucose, T4, creatinine and blood pressure.
- Start a 3 day log with 6 fields per event and a blacklight pass each night.
- Cut the lip to 5 cm or less this weekend, or add a 12 cm doorway.
- Add 1 tray per floor and keep every tray within 5 m of a sleeping spot.
- Switch to 3 to 5 cm of unscented litter, scoop twice a day, and clean old spots with an enzyme product.
If you are rebuilding the whole bathroom corner, the sizes and placement rules are collected in the litter box setup guide, which pairs well with the 3 day log above: change 1 variable at a time, or you will not know which fix worked.

Frequently asked
Why is my 13 year old cat suddenly urinating next to the litter box?
Start with a vet visit, not a new tray. Roughly 40 percent of senior house soiling has a pain or medical driver, and joint disease is common past 12. Ask for urinalysis with specific gravity, culture, glucose, T4, creatinine and blood pressure, then fix the 15 cm lip and any tray more than 5 m from the bed.
How close should a litter box be for an old cat?
Within 5 m of every sleeping spot, on every floor, with 1 more tray than the number of cats. An arthritic cat will not walk 12 m and down a flight of stairs at 3 am; it will use the floor at 2 m. At least 1 tray in the house should be uncovered.
How do I know if it is incontinence or behaviour?
Incontinence leaks while the cat sleeps or drips without squatting, and it needs a separate workup. A male cat that crouches for 5 minutes and passes 0 to 3 drops is an emergency within 12 hours, because a urethral plug can be fatal in 24 to 48 hours.
