Pica in a cat over 11 is rarely a discipline problem. Three drivers cover most cases: a medical condition the cat cannot describe, a diet that is short on something it needs, and a brain or a room that stopped being interesting. The work is 2 weeks of recording (14 days), 4 exclusion checks and 2 changes at home. Some swallowed items become surgical emergencies inside 24 hours, so triage comes before training, and no supplement or drug belongs in the bowl without a vet prescription.

The 3 drivers, in the order vets check them

Start with the body. Dental pain is the most under-diagnosed driver in old cats: resorptive lesions and fractured molars make chewing hurt, and a cat that cannot chew kibble may lick and chew plastic instead. Gastrointestinal disease, hyperthyroidism, chronic kidney disease and anemia all move appetite, and any of them can push a cat to eat litter or fabric. Thiamine deficiency is uncommon but real in cats fed raw fish or a badly stored diet. Then the bowl: a diet at 30 kcal per kg when the cat needs 50 to 60 kcal per kg leaves an old cat hungry for 22 hours a day, and a fast switch to a high-fiber weight diet does the same. Then the head: cognitive dysfunction, boredom, and stress from a new pet or a moved box produce identical mouth behaviour, and wool sucking that began at 2 or 3 years often persists into old age.

14 days of records a vet can use

Two weeks is the shortest window that separates a one-off from a pattern, because appetite and activity in old cats swing on a 3 to 5 day cycle. Log 5 fields per episode: date and clock time, the item, how much was swallowed, vomiting or stool within 24 hours, and a 0 to 3 severity score. Photograph the item next to a coin and keep every photo in 1 album, so 8 photos from 8 days can be compared side by side. Weigh the cat every 7 days on the same scale: a 4.5 kg cat that drops 200 g has lost 4.4 percent, and at that point the weight chart matters more than the pica chart. If the target is litter, note which box and whether it clumps.

The 4 exclusions that close the medical half

One: blood panel, total T4 and urine specific gravity in a single visit, with results in 24 to 48 hours, which covers kidney, thyroid, liver and anemia causes at once. Two: a dental exam with dental x-ray under sedation, because resorptive lesions sit below the gum line and cannot be seen from above. Three: abdominal imaging, x-ray or ultrasound, if there is vomiting, a 5 percent weight loss, or a mass the vet can feel. Four: systolic blood pressure taken twice, 5 minutes apart, where 160 mmHg is the usual flag. Run all 4 before buying a single supplement, because a supplement given for a dental abscess wastes 4 to 8 weeks and 1 more check.

The 24 hour line

Go now, not tomorrow, for string, tinsel, ribbon, dental floss, hair ties or any linear item: a linear foreign body can cut through bowel in 24 to 48 hours, and a 300 dollar endoscopy becomes a 2,000 dollar resection. The same clock applies to 3 vomits in 12 hours, drooling, 24 hours without food, a hard painful abdomen, or 36 hours without stool. Pica with tremors, head pressing, circling or a seizure is a same-day visit. Clumping litter eaten in handfuls can set like cement in the colon: 1 episode is a phone call, 3 in a week is an exam.

2 environment fixes for week 1

Removal is half the job; the other half is a legal substitute within 1 metre of the old target. Put every elastic, ribbon and bag into a lidded bin at 1.2 metres, then place 3 acceptable chews: a puzzle loaded with 10 to 15 kibble, a silvervine toy, and a pot of cat grass. The second fix is feeding: 4 to 6 small meals a day from 2 puzzle feeders and 1 timed feeder, so the mouth has work that pays. Add 2 play sessions of 6 to 8 minutes at 8 a.m. and 9 p.m., 1 perch at 1.5 metres, and 2 boxes, and the cat has 5 better options than the carpet.

When the driver is cognitive

If pica arrived with night yowling, staring at a wall, soiling outside the box and getting stuck in a familiar corner, treat it as cognitive dysfunction until proven otherwise. Score 4 signs every 30 days: night waking, disorientation, changed social behaviour, house soiling. Two or more signs, present for 4 weeks, with the 4 checks clear, is enough for a vet to discuss management. Diet, enrichment and prescription drugs are the 3 levers; the drugs are prescription only, dosed by weight and liver values, and never started from a blog post.

Water and weight: the 2 numbers to watch

An old cat that eats litter often drinks less than it should, and intake under 40 ml per kg per day points at the gut rather than the brain. Measure the bowl instead of guessing: 200 ml gone in 24 hours from a 4.5 kg cat is about 44 ml per kg, which is normal. A fountain that runs 24 hours a day lifts intake in many cats within 1 to 2 weeks, and the setup costs less than 1 ultrasound. The routines for both numbers are in the fountain guide and the weight log guide.

First 7 days, in order

  • Day 1: photograph and remove every string, elastic and bag; bin at 1.2 metres.
  • Day 1 to 2: start the 5 field log and the 0 to 3 score; 1 page per day.
  • Day 2: book 1 visit that covers blood, T4, urine and blood pressure.
  • Day 3: switch to 4 to 6 meals a day, 2 puzzle feeders, 1 timed feeder.
  • Day 5: add 2 play sessions of 6 to 8 minutes and 1 perch at 1.5 metres.
  • Day 7: weigh, compare with day 0, and call the vet if the drop is 5 percent or more.

Expect the log to look worse before it looks better: 3 to 5 episodes a week is common in the first 14 days, and the count is the baseline, not a failure. A cat that stops eating fabric but starts eating litter has not improved; it has changed target, and the 4 checks are still the answer. Total cost of the workup runs about 150 to 400 dollars at most clinics, which the cost calculator can break down by test.

Frequently asked

How do I know if my old cat's pica is an emergency?

Any linear item such as string, tinsel or floss is an emergency within 24 hours, because it can saw through bowel. So is 3 vomits in 12 hours, drooling, 24 hours without food, a hard painful abdomen, or 36 hours without stool. Call the clinic before you feed anything or induce vomiting.

Will a supplement stop a cat eating litter?

Rarely on its own. A supplement only helps if a specific deficiency is confirmed, and most old cats with pica have dental pain, gut disease, thyroid disease or anemia instead. Run the 4 checks first, because 4 to 8 weeks on an unnecessary supplement delays the real diagnosis.

How long before an enrichment plan shows a change?

Give it 2 to 4 weeks with the same 5 field log. Feeding 4 to 6 small meals, 2 play sessions of 6 to 8 minutes and 3 acceptable chews usually cut episodes by half in that window when no medical cause is present.
