A cross-eyed Siamese is usually not a sick cat. Convergent strabismus in the pointed breeds comes in 2 forms: a wiring difference present from the first 8 weeks, and a new turn in an older cat that is a sign of something else. The first needs watching, the second needs a veterinarian within 24 hours, and neither shortens a normal lifespan.

Mechanism 1: the eyes are wired differently

In Siamese, Himalayan, Balinese, Tonkinese and Colorpoint Shorthair lines, the same temperature-sensitive pigment pathway that produces the dark points also alters how optic nerve fibres sort themselves at the chiasm. More fibres cross to the opposite hemisphere than in a normally pigmented cat, so each side of the brain gets less input from the eye on its own side. Binocular overlap shrinks, part of the depth map is lost, and many of these cats hold one or both eyes turned inward by 5 to 15 degrees to recover a usable field.

The turn appears between 6 and 8 weeks, when kittens start tracking a moving toy, and it is symmetrical in most cases. It does not progress after 6 months, it is not painful, and the eye itself is healthy.

Mechanism 2: an adult cat that turns suddenly

A turn that appears after 12 months is a different problem with a different list: vestibular disease, which brings a head tilt of 15 to 45 degrees and nystagmus; orbital inflammation or trauma after a fight or a fall; lens luxation; thiamine deficiency in a cat on a poor or fish-heavy diet; myasthenia; and raised pressure inside the skull. Here the strabismus is one of 3 or more signs, and the clock matters: 6 to 12 hours for a sudden turn with a head tilt, 24 hours after trauma, 48 hours for a slow drift with no other sign.

3 observations you can make at home

1. The photo test, 4 shots. Stand 1 m away, camera at eye level, flash on. Compare each pupil against the inner corner of its eyelid. Under 1 mm reads as straight, 2 mm shows in a photo, and 3 mm is what most owners notice in person first.

2. The cover test, 30 seconds. Cover one eye with a flat hand for 3 seconds, watch the uncovered eye swing to fixate, then swap. A swing of more than 2 mm as the cover comes off is a turn, and its direction says convergent or divergent.

3. The head-position check, 60 seconds. Watch the cat eat for 1 minute, then jump onto a 40 cm surface. A head held at 10 to 15 degrees, or one eye squinting while eating, means the cat is compensating for a field it cannot see.

Three findings move this from observation to a same-day call: a head tilt visible in a photo, nystagmus at rest, or pupils of 2 different sizes.

Read column 3 rather than column 1. A turn that has not moved in 3 months is the congenital form whatever the age; one that moved inside 72 hours is the acquired form whatever the age. Age sets the odds, movement sets the urgency.

What the vet checks in 20 minutes

Vision by menace response in each eye, pupillary light reflex, fluorescein stain for corneal ulcer, a Schirmer tear test (15 to 25 mm per minute is the usual range), a fundus exam, and the squint measured in 2 gaze positions. A cat over 8 years also gets blood pressure, with 180 mmHg systolic as the figure most clinics treat as the cut-off. The Exotic Shorthair carries its own eye set, covered at the Exotic Shorthair eye page, and a turn with discharge belongs with the discharge guide.

Does it shorten life, or take vision

No, and partly. A cross-eyed Siamese at 12 years is an ordinary patient, and the congenital form is not linked to a shorter life in these breeds. What is reduced is stereopsis, the close-range depth sense built from 2 overlapping images. A jump onto a 30 cm shelf may be misjudged by 5 to 10 cm, catching a toy in the air is less accurate, and the cat leans harder on whiskers, hearing and motion.

Four cheap changes follow: shelves at least 30 cm wide so a missed landing still lands; a night light in each room used after dark; food and water 8 cm off the floor and against a wall; and toys dragged along the floor rather than thrown.

Breeding, screening and what not to do

The trait is polygenic, so there is no DNA test to order. Breeders commonly photograph the face at 8 weeks and again at 12 months and avoid pairing 2 cats with a marked turn, because 2 turned parents produce turned kittens far more often than one. For a pet, the useful screening is the 30-second cover test at every annual visit plus a photo every 6 months, so a 1 mm drift becomes a number.

Surgical correction exists and is rare, a specialist decision made for a functional reason, not a cosmetic one. No diet, supplement or exercise has been shown to straighten the eye.

When to stop watching and call

Call the same day for a turn that appeared inside 72 hours, a head tilt, nystagmus, unequal pupils, a turn with discharge or squinting, or a cat that starts bumping into furniture it used to clear. If a household product got into the eye, rinse with saline for 5 minutes and check the toxicology list before deciding to wait until morning.

5 steps for this week

  • Take 4 flash photos at 1 m and keep them in one folder as the baseline.
  • Run the 30-second cover test today and write the result in millimetres.
  • Photograph the cat eating, so a 10 degree head tilt has something to compare against in 6 months.
  • Widen the 2 narrowest shelves to 30 cm and add a night light to the busiest room.
  • Write down the onset date; a turn present since 8 weeks is information the vet needs in the first 30 seconds.

Frequently asked

Will a cross-eyed Siamese kitten grow out of it?

Usually not, and it does not need to. The congenital form is set by 6 to 8 weeks, stays within 1 to 2 degrees after that, and does not affect comfort or lifespan. What is worth doing is a photo at 1 m every 6 months, so a genuine drift of 2 mm or more is caught early enough to be examined rather than argued about.

Can two cross-eyed cats have straight-eyed kittens?

They can, because the trait is polygenic and not a single switch, but the odds are worse than with one turned parent, which is why breeders avoid pairing 2 markedly cross-eyed cats. There is no DNA test for the squint itself, so the screening that exists is photographic at 8 weeks and again at 12 months.

When is a new squint an emergency?

A turn that appears inside 72 hours in a cat over a year old, especially with a head tilt, nystagmus, unequal pupils or a drooping lid, points at vestibular, orbital or central disease and needs a veterinarian within 6 to 24 hours. A turn stable since kittenhood is a different problem and can wait for a routine visit.
