Treat every swallowed length of floss as a 24-hour problem. In the first 24 hours a linear foreign body is usually still operable with a good outcome; by 48 hours the strand can have sawed through the intestinal wall and the cat is in septic peritonitis. The rule is one line long: no pulling, no cutting, no inducing vomiting, and a vet visit inside 24 hours rather than a wait for symptoms.

The temptation to intervene comes from how harmless the object looks. A 45 cm floss pick and a 4 cm elastic hair tie are the 2 most common feline intestinal foreign bodies in general practice.

Two categories, two different clocks

Linear material — floss, sewing thread, tinsel, ribbon, fishing line — anchors itself. One end catches under the tongue or at the pylorus, peristalsis keeps pulling, and the intestine gathers like a curtain on a cord. The wall is cut by pressure, not by sharpness, which is why a soft 45 cm thread does the same damage as a needle.

Elastic loops — hair ties, rubber bands, the elastic from a sock — behave differently. They are too short to anchor at both ends, so they obstruct rather than plicate: the loop sits at the pylorus or in the jejunum and blocks. Obstruction declares itself in 24-72 hours with vomiting that gets more frequent, not less.

The 24-hour line, and what it changes

At hour 24, a linear foreign body normally means surgery the same day, with a good chance of full recovery and a 3-5 day stay. Between 24 and 48 hours the risk of perforation climbs steeply and the operation turns from a simple enterotomy into a resection with a contaminated abdomen. Past 48 hours, the cat that was eating on day 1 is often the cat in shock on day 3.

That is why the decision is not based on how the cat looks. Cats compensate well and hide it: a cat with a plicated gut can eat a full meal at hour 20 and be flat at hour 40. Book on day 1 and cancel if imaging is clean.

The 10-second mouth check, 3 places

Lift the upper lip and look at the outside of the molars. Lift the tongue and look at the 2 folds underneath, where floss most often catches. Then open the jaw slightly and look at the throat and the ridge behind the last molar.

If you see a strand, stop. Do not tug it to test whether it is attached; a 2 cm pull can slice the tongue base or tear the bowel at the far end. Note the color and length, photograph it if you can, and go.

Why no pulling, no cutting, no vomiting

Pulling converts a contained strand into a saw. The far end may be anchored at the pylorus, and 500 g of tension is enough to cut mucosa. Cutting the visible end removes your only marker of what the cat swallowed, and the remainder retracts out of reach, so the vet loses the length estimate that guides the decision.

Inducing vomiting fails for the same mechanical reason: the strand is anchored at both ends, nothing comes up, and the retch drives the loop harder into the gut wall. Emesis is a decision for a vet who has examined the cat, never a home measure.

What happens in the first 60 minutes at the clinic

The vet takes a history of material, length and time, then examines the mouth and abdomen. Imaging comes next: plain X-rays show hair ties with metal joins and the gas pattern of obstruction, but floss and plain elastic are radiolucent, so ultrasound is what actually finds a linear foreign body.

If the cat is stable and the imaging is equivocal, the usual plan is 12-24 hours of observation with a repeat scan, not discharge. Surgery, if needed, runs 2,000-6,000 RMB depending on whether a resection is required.

The second row is the one that ends badly. A cat that unspools a 15 m container has swallowed metres of line, and that is a same-hour emergency regardless of how normal it looks.

Hair ties: the elastic exception

Hair ties are small enough to pass, and many do, which makes them dangerous: the owner waits 3 days because the last one passed. A 4 cm loop that does not pass lodges at the pylorus, and the cat vomits progressively more often over 24-72 hours while still drinking.

If a hair tie is missing and the cat has vomited twice in 24 hours, that is enough to justify imaging.

Count, clear, contain

Count your hair ties and floss picks once a week and write the number on the container. Missing 1 of 12 is a data point; missing 1 of an unknown number is nothing. Sweep a 2 m radius around the bed and the bathroom basin, because that is where both objects live and where cats patrol.

Put floss in a drawer with a catch, not on the basin, and hair ties in a closed jar rather than on the bedside table. One elastic or a 30 cm offcut on the floor is the entire exposure.

Go now, not tomorrow

Go the same hour for: vomiting twice in 12 hours, a visible strand at the mouth or anus, a spool or a needle involved, or a cat that cries when the abdomen is touched. Go within 24 hours for any confirmed swallow with no symptoms at all. Do not wait for blood, and do not wait for the cat to stop eating, which is a late sign.

Coins, batteries and zinc items run on a second clock; the metal cases are covered in the zinc and coin guide and in the toxin index.

Five steps if this just happened

  • Look in the mouth for 10 seconds, then stop touching it.
  • Write down the material, the length and the time of ingestion.
  • Remove the rest of the supply from the room and count what is left.
  • Call the vet, describe all 3 details, take the earliest slot inside 24 hours.
  • Offer no food after the call, and go in immediately if the cat vomits twice.

Frequently asked

My cat swallowed 40 cm of floss 6 hours ago and is eating. Can I watch it?

Watching is the wrong verb, because the damage is mechanical and silent for the first day. Book an appointment inside 24 hours, keep the floss container so the vet can see the material, and check the mouth and under the tail every 8 hours. If the cat vomits twice in 12 hours, go the same hour instead.

Should I make my cat vomit after it eats a hair tie?

No. An elastic loop is usually too large to come back up, the retching pushes it harder into the stomach wall or pylorus, and there is an aspiration risk. Emesis is a decision for a vet who has examined the cat. Keep the cat calm, note the size and count, and phone the clinic.

Why can a soft thread cut through intestine?

Because the gut does the cutting. One end anchors under the tongue or at the pylorus, peristalsis keeps tension on the strand, and the intestine folds around it like fabric on a drawstring. The folds press on a 1-2 mm line of wall until it necroses, usually in 24-48 hours, which is why softness is irrelevant.
