A sting inside a cat mouth reaches peak swelling in about 15 minutes, and the decision belongs to that window: drive now, do not watch. Three signals mean the car immediately — a voice that changes, a neck stretched forward with elbows out, and gums that turn pale or grey. On the way, do not put fingers in the mouth, do not syringe water, do not give human antihistamine tablets, and do not wait for the swelling to go down on its own.
Why a mouth sting is not a paw sting
Bee venom is a protein mix that opens capillaries. On a paw, 0.05 mL of venom under thick skin gives a 1-2 cm swelling that hurts for a day. On the tongue, the hard palate or the floor of the mouth, the same 0.05 mL sits in 2-3 mm of loose tissue over a 5-6 mm airway. One millimeter of swelling at the base of the tongue can narrow the opening by 30%. Two to three millimeters turns breathing into a whistle.
The tongue has nowhere to swell outward, so it swells down into the throat. A cat mouth holds about 2 mL of saliva before it starts to drip, which is why 5 mL of drool means swallowing is already compromised.
Where the sting lands changes the plan completely: a sting on the throat can close the airway inside 10 minutes, a sting on the face usually swells for 2-3 days without touching breathing, and the toxicity checker is for the times you are not certain it was a bee at all.
The 3 airway signal groups
Sound. A purr that becomes a whistle, a meow that loses its second syllable, or 1 breath loud enough to hear from 1 m away.
Posture. Neck stretched forward, elbows out from the body, head carried below the shoulders, refusing to lie flat or be picked up.
Colour. Gums that go from pink to pale, capillary refill over 2 seconds, or a tongue turning grey-blue at the edges.
Any 1 of the 3 ends home care. A resting breathing rate above 40 per minute in a cat that was at 24-30 an hour earlier is a fourth check, and a cat that starts open-mouth breathing has an opening of roughly 3-4 mm left.
The first 60 minutes, in 15-minute blocks
| Clock | What you see | What to do | Number to watch |
|---|---|---|---|
| 0-2 min | Pawing the mouth, 1-2 mL of drool | Look once with a torch, then close the mouth | Stinger is 2-3 mm, usually on the palate |
| 2-5 min | Swelling visible, voice still normal | Ice under the jaw, chin only | 5 min on, 5 min off |
| 5-15 min | Swelling at peak, 1-2 cm | Carrier, keys, call the clinic | 15 min to peak swelling |
| 15-30 min | Drooling 5 mL, swallowing 3-4 times | Drive, no water, no food | 3 ice cycles is the ceiling in hour 1 |
| 30-60 min | Voice changed or breath audible | Arrive, say mouth sting and the time | 40 breaths per minute at rest |
Ice in 5-minute cycles, and no longer
Wrap a gel pack in a thin cloth, press it under the jaw and along the cheek, and never across both nostrils. Hold 5 minutes, rest 5 minutes, and stop at 3 cycles in the first hour. A 4 kg cat loses heat quickly through the chin, and a body temperature under 37 C adds a second problem to the first. Ice will not shrink the swelling that matters inside the throat; it keeps the cat still and takes the edge off, which is worth 5 minutes at a time and not more.
Stinger: scrape, never pinch
A honeybee leaves a 2-3 mm stinger with a venom sac attached. A wasp leaves nothing and can sting 3-4 times in the same mouth, which is why 3 or more stings in one cat changes the risk by a lot. If the stinger is visible on the palate or the side of the tongue, scrape it out in 1 direction with the edge of a card. Pinching pushes the remaining 30-50% of the sac into the tissue. If you cannot find it in 5 seconds, stop looking and drive.
What the clinic does that you cannot
Adrenaline for airway swelling, injectable antihistamine, a steroid and oxygen are all prescription drugs, dosed by weight, heart rate and whatever else the cat is taking. Human antihistamine tablets are the most common home mistake: the tablet is sized for a 60 kg person, the cat is 4 kg, and no tablet removes a 2 mm swell from a 5 mm tube.
Most mouth stings that reach a clinic within 30-60 minutes get 1 injection, 2-6 hours of observation and soft food for 24-72 hours. Cats that arrive with a changed voice are kept longer, because the swelling can rebound 4-8 hours after the first injection.
The 0-1-2-6-24 hour view
- Hour 0: the sting lands, swelling runs to its peak by minute 15, and the car should already be moving.
- Hour 1: swelling at maximum, 5 mL of drool, and the voice is the number that matters.
- Hour 2: after 1 injection the swelling should be level, not climbing.
- Hour 6: the rebound window opens. A voice that changes again at this point goes back to the clinic.
- Hour 24: swelling down by half, soft food accepted, and the 2-3 mm ulcer still 2 days away.
A cat that is quiet at hour 2 and hoarse at hour 6 was not over-treated at hour 2; the venom was still working through the tissue.
The 24 hours after
- Swelling should be about half its peak at 24 hours and gone by 48-72 hours.
- Drooling should stop within 6-12 hours; drool that continues past 12 hours means the tongue is still too swollen to swallow.
- A 3-5 mm grey ulcer where the stinger sat is normal sloughing on day 2-3. A growing, hot, foul-smelling lump is not, and needs a recheck.
- Rectal temperature above 39.5 C, or refusing food for 24 hours, sends the cat back in.
- Keep the cat indoors for 24 hours: a second sting on a still-swollen mouth is far worse than the first.
4 steps that take 5 minutes
- Close the windows and find the insect. A wasp indoors can sting the same cat again within seconds.
- Look once, 5 seconds, with a torch — palate, tongue sides, inner cheeks — then leave the mouth alone.
- Ice 5 minutes on, 5 off, under the jaw, while someone calls the clinic.
- Carrier, car, and 2 facts on arrival: mouth sting, and the time it happened.
Fifteen minutes is short enough that the plan has to exist before the sting. Keep a gel pack in the freezer, a card in the kitchen drawer and the clinic number on the fridge, and the whole sequence fits inside the window where it still changes the outcome.

Frequently asked
How fast does a bee sting in a cat mouth become dangerous?
Swelling peaks in about 15 minutes, and the airway is only 5-6 mm wide, so a 2-3 mm swell is already serious. Do not plan to watch for an hour: if the voice changes, the neck stretches forward, or the gums pale, drive immediately.
Should I give my cat an antihistamine for a mouth sting?
Not without a vet. Human tablets are dosed for a 60 kg person, they do nothing for a 2 mm airway swell, and some ingredients are unsafe for cats. Prescription injectable antihistamine, a steroid or adrenaline are clinic decisions made by weight and heart rate.
The swelling went down in an hour. Do we still need the vet?
Yes, ideally the same day. Swelling can rebound 4-8 hours after the first injection, and a sting site on the tongue can slough into a 3-5 mm ulcer by day 2-3. A recheck also rules out a retained stinger and secondary infection.
