A cat adopted 3 to 10 days after surgery is not a sick cat, but it is a cat with a 5 cm incision, a shaved flank and a drug schedule someone else started. The first 14 days run on 3 checks and 5 red lines, and the 4 stages below tell you how much handling, space and food to allow on each day.
Ask for the discharge sheet before you sign: the operation name, the date, the suture type, the last pain dose and the recheck date. A cat whose sheet says 10 to 14 days of sutures and a recheck on day 12 needs a calendar, not intuition.
The 0-14 day table
Four stages, each with a handling cap and a food number. Move up a stage only when the previous one is met for 48 hours, not when the cat looks bored.
| Days | Handling per day | Space | Food | Call the vet if |
|---|---|---|---|---|
| 0-2 | 2 sessions of 3 minutes, no lifting the middle | 1 room, 4 sq m, box 1 m away | 4 meals of 20-30 g wet food | No food in 24 hours |
| 3-5 | 3 sessions of 5 minutes, harness hold only | 1 room plus a 2 m corridor | 4 meals of 30-40 g, water measured | No stool in 48 hours |
| 6-9 | 4 sessions of 5 minutes, short lap time | 2 rooms, all jumps blocked | 3 meals of 40-50 g plus 1 snack | Incision redness over 1 cm |
| 10-14 | Free handling with a 10 minute cap | Whole flat, no stairs for 7 more days | Normal portion, weighed weekly | Suture line opens at any point |
Check 1: the incision, 5 cm at a time
Look once in the morning and once at night, and photograph it on the same phone at the same distance. What is normal on day 1 is a line that is slightly pink at the edges, dry in the middle, and 5 to 8 cm long depending on the operation. What is not normal is a 1 cm ring of redness that was not there yesterday, a discharge that stains the gauze yellow-green, or a gap you can see between 2 sutures.
Do not clean the line with anything stronger than what the discharge sheet names, and do not let the cat lick it for 10 to 14 days. A cone or a soft collar is the tool; a cat that gets the collar off in 20 minutes needs a body suit instead. Swelling that doubles the width of the line in 24 hours is a same-day call, not a wait-and-see.
Check 2: grams in, milliliters out
Weigh the food, do not eyeball it: a cat that eats 25 g instead of 45 g looks fine under a blanket. Water counts best in a marked bowl: a 4 kg cat often wants 180 to 240 ml, at 45 to 60 ml per kg.
Output matters as much as input. A cat that has not urinated in 12 hours needs a call, and a male cat at 8 hours needs one faster, because urethral obstruction after anesthesia is a same-night emergency rather than a tomorrow problem. Stool can lag 48 hours after surgery; beyond that, ask about the pain drug, which slows the gut.
Check 3: a 4-item pain score, 0 to 3 each
Pain in cats is quiet, so score 4 things instead of trusting the face: posture (0 flat and relaxed, 3 crouched with a tucked belly), ear position (0 forward, 3 flat and rotated out), response to a light touch 5 cm from the incision (0 ignores, 3 flinches and hisses), and time spent hiding (0 out in the open, 3 under furniture for 6 hours).
A total of 4 or more, or any single 3, means a vet call that day. Do not add or change a pain drug yourself, and do not split a tablet to make it last: the dose on the label is set for this cat's weight, and the timing is set for its liver and kidneys.
The 5 red lines
- The incision opens, or leaks pus that smells. Cover it with a clean pad and go in, same day.
- No food for 24 hours, or vomiting 3 times in 12 hours.
- No urine for 12 hours, or 8 hours for a male cat, or any cat straining in the box with nothing coming out.
- Temperature under 37.5 C or over 39.5 C, taken rectally once and confirmed once.
- Breathing over 40 per minute at rest, gums pale or grey, or the cat cannot be roused.
Any 1 of the 5 overrides the schedule. A cat that meets 1 red line at 23:00 goes to an emergency clinic, and the 200 to 600 RMB of an after-hours visit is cheaper than a repair at 3 times the price.
Days 10 to 14: sutures, cone, first recheck
Sutures come out at 10 to 14 days, and the recheck should already be booked for day 10 to 12 so a slow line gets 2 more days rather than a second visit. Ask the vet to check 3 things at that visit: the line, the weight against the day 0 number, and the range of motion in the leg or the jaw, depending on the operation.
The cone comes off when the vet says so, not when the line looks closed, because a cat can open a 14-day incision on day 9 with 30 seconds of licking. Stairs stay blocked for 7 days past suture removal, and jumping onto a 1.2 m counter stays blocked for 14; a cat that lands short once can undo a flank repair.
What surgery adds to the yearly bill: 200-600
The ongoing cost is smaller than most adopters fear. Expect 200 to 600 RMB a year on top of a normal cat budget: 1 recheck at 150 to 300 RMB, a joint or weight supplement at 100 to 250 RMB, and 0 to 200 RMB for a prescription diet if one is needed. A cat with a plate or a repaired fracture sits at the top of that range; a cat 4 weeks past a spay sits at the bottom.
Budget the first month separately, because it is the expensive one: 300 to 800 RMB for the recheck and a cone, plus 200 to 500 RMB for a taller box and a ramp. The cost tool splits setup from monthly, and a 30-day foster period, which the foster-to-adopt route sets up, moves the surgical risk to the shelter for the window that matters most.
Your 5 steps for day 0
- Put the cat in 1 room of 4 sq m with the box 1 m from the bed and no jump over 40 cm.
- Write the 6 discharge numbers on the fridge: operation, date, sutures, last dose, recheck, clinic phone.
- Start the log: food grams, water ml, urine, stool, pain score, 2 times a day.
- Screen the room before the cat explores it: the window and screen check covers the 2 fall risks that reopen incisions.
- Book the recheck for day 10 to 12 and put the 5 red lines on the same page.
Frequently asked
How much should I handle a cat after surgery?
Two sessions of 3 minutes a day for the first 2 days, then 3 sessions of 5 minutes from day 3, always supporting the chest and belly rather than lifting from the middle. By day 10 to 14 handling is free with a 10-minute cap. Move up a stage only after 48 hours at the previous one.
When is hiding a sign of pain rather than fear?
Use the 4-item score: posture, ear position, response to a touch 5 cm from the incision, and hours spent hiding. A total of 4 or more, or any single 3, means a vet call that day. Hiding alone is not the signal; hiding plus a tucked belly and flat ears is.
What does a post-surgery cat cost per year?
About 200 to 600 RMB more than a cat with no surgical history: 1 recheck at 150 to 300 RMB, a supplement at 100 to 250 RMB, and 0 to 200 RMB for a prescription diet. A cat with a plate sits at the top of the range, a cat 4 weeks past a spay at the bottom.
